Overview
Schedule 15 is the CRA form that reports your FHSA contributions, transfers, and activities on your tax return. It helps calculate your FHSA deduction, tracks unused FHSA contributions you may be able to deduct in a future year, and lets the CRA know when you've opened your first FHSA.
Wealthsimple Tax completes Schedule 15 for you based on the FHSA information you enter. To learn how to enter it, see Report FHSA activities on your tax return.
When you need Schedule 15
Schedule 15 is required if any of the following applied to you during the tax year:
- You opened your first FHSA
- You made contributions to your FHSAs
- You made a qualifying, designated, or taxable withdrawal
- You made a designated transfer
- You're claiming an FHSA deduction
- You became a successor holder of an FHSA
- You transferred property from your RRSPs to your FHSAs
If none of these apply to you, you don't need Schedule 15.

Frequently asked questions
Why is Schedule 15 missing from my return?
Schedule 15 only appears when your FHSA information requires it, such as when you've opened your first FHSA, entered a T4FHSA slip, or claimed an FHSA deduction. You can check your FHSA information in two places in Wealthsimple Tax:
- Other stuff we have to ask: Check your answer to the question "Did you open your first First Home Savings Account (FHSA) in [YEAR], or become a successor holder in [YEAR] and did not open another FHSA of your own?" If you answered Yes, Schedule 15 is included on your return.
- Manage tax forms: In the left sidebar, go to Manage tax forms > T4FHSA slips. If you've entered a T4FHSA slip in the FHSA Contributions & Income section, Schedule 15 is included on your return.
If you didn't open your first FHSA, enter a T4FHSA slip, or claim an FHSA deduction, Schedule 15 isn't required.
