To report income benefits from Workers’ Compensation, Social Assistance Payments, or Provincial Supplements, type “T5007” in the search box and add the Statement of Benefits - T5007 section to your return.

- Report Social Assistance Payments and Provincial/Territorial Supplements in box 11.
If you have linked your return with your partner, and have an amount in box 11, you must answer the question “Did you live with your partner when you received these social assistance payments?”. If the answer is “Yes”, that income will only be included on the higher income partner’s return.
